NextStepInCare.org provides information and advice to help family caregivers and health care providers plan safe and smooth transitions for patients.

They offer free, downloadable guides and tools for delaing with transitions, which are times when the care surrounding a loved one is a loved one is at its most vulnerable and often resulting in re-hospitalizations and caregiver exhaustion.

These tools help caregivers navigate transitions with clearer communication and beter coordination between themselves, loved ones and providers in various health care settings.
